Catholic High School Religion Teachers/Catechists Certification

 Certification Reporting form

When completing the Certification Reporting form, in the Academic Work Section, record your degree OR record your course work/equivalent hours (workshops, etc.) in the Competency Area (Part B) on the form. Your supervisor (principal or religion department head) signs and dates the Internship Box when the internship requirements have been completed.

If your course work and internship is complete:

• Return the completed form with all relevant supporting data (transcripts, certificates, etc.) to the Office for Faith Formation/Catechetics – 305 Michigan Ave. – GRB6 – Detroit, MI 48226. Attn: High School Catechist Certification

• The Office for Faith Formation will evaluate the information you have provided. You will be sent a certificate acknowledging completion of the requirements (academic and internship).

• The Office for Faith Formation will send a copy of your certificate to the Office for Catholic Schools form your personnel file.

• It is your responsibility to provide a copy of the certificate to your school principal for your local personnel file.

If you have not yet finished all the course work or experience to complete the form:

• Return the incomplete form with all relevant supporting data (transcripts, certificates, etc.) to the Office for Faith Formation/Catechetics – 305 Michigan Ave. – GRB6 – Detroit, MI 48226. Attn: High School Catechist Certification

• The Office for Faith Formation will evaluate the information you have provided. You will be sent a letter noting that you are in the process of obtaining certification and listing the requirements that are needed to complete the certification process.

• The Office for Faith Formation will send a copy of your "in process" letter to the Office for Catholic Schools form your personnel file.

• It is your responsibility to provide a copy of the "in process" letter to your school principal for your local personnel file.

• At the end of each school year, update your certification form by sending supporting data for new courses, workshops, etc. Upon completion of all certification requirements, send completed form to the Office for Faith Formation/Catechetics – 305 Michigan Ave. – GRB6 – Detroit, MI 48226. Attn: High School Catechist Certification
